Install and activate Affiliate for WooCommerce plugin and Contact Form 7 or CF7 plugin, and WooCommerce, ofcourse.

With this Contact Form 7 addon, you will see eight new form tags ( reference ) for affiliate registration. These tags, each with a prefix ‘Affiliate Registration’ allows you to add all the default affiliate registration fields:
- Email Address
- First Name
- Last Name
- Password
- Confirm Password
- PayPal Email Address
- Referral affiliate user ID
- Website URL
But go beyond and customize the form to collect crucial details to assess each affiliate’s potential:
- Phone number / WhatsApp number
- Social profiles
- Audience size
- Industry or niche
- Promotional methods (blogs, emails, social, ads)
- Bank details
- Tax details – VAT or GST number
- Mailing address and country
- Reasons to join your affiliate program
- Terms and conditions

Atleast make email, first name and last name, password, terms and conditions mandatory without fail.
By gathering all the additional information, you can better evaluate the affiliates’ ability to promote your brand effectively.
